TATTOO SPECIAL BOOKING FORM

Please make sure to read through this form carefully before submitting. This ensures that you are aware of the booking process, guidelines and how to be prepared for your session. Should you be selected, my response will be sent to you by SMS.

PLEASE READ THE FOLLOWING:

  • Cash is preferred but not mandatory.

  • Please come on time. If you are late I will proceed to the next person in queue. Lateness will affect the entire tattoo day for all clients.

  • Those that are selected for the day will be notified of their number in queue with further information.
